*Please note the CDC recommends masks be used only for children aged 2 and above.  It is our policy to make masks based on CDC recommendations.

                                     Due to the overwhelming demand for Face Masks, 

                          our maximum allowable request at any one time is 100 masks.  

PLEASE NOTE:  Any request for over 50 Face Masks must fill out TWO FORMS...

 1.  this MASK REQUEST FORM below, AND... 

 2.   a  DUE DILIGENCE FORM pertaining to the use of the Face Masks before those requests will be input into our system. After you fill out the Mask Request Form, you can return to this page and click on the link below for the Due Diligence Form. )

Please fill out the entire form below to request masks. Due to the large volume of requests we RECEIVE, we cannot guarantee your request will be fulfilled, but we will add you to our system and do what we can.


We are a group of volunteers committed to putting our best sincere effort forward in reviewing and disseminating information. We are not medical professionals nor attorneys, and do not have knowledge of medical or legal requirements. Our understanding and belief is that the CDC permits the use of reusable cloth face masks in a crisis response when there are absolutely no other options available. A reusable face mask is NOT considered PPE since per the CDC, “their capability to protect healthcare personnel is unknown.” A reusable face mask is not stand alone safety equipment and for healthcare workers, per CDC should ideally be combined with a face shield. For both healthcare and community members, they should also be used in conjunction with and is only to be used in conjunction with social distancing, frequent hand washing, social distancing, and other safety procedures. It does not provide the wearer with reliable protection from inhaling smaller airborne particles and is not considered respiratory protection.   It should NOT be used in a surgical setting or where significant exposure to liquid bodily or other hazardous fluids may be expected.Please exercise your personal judgment and discretion and make your own decisions regarding the use of reusable face masks, as it should be a personal educated decision and information from this page is NOT medical advice and should not be relied upon as such. Please note that these are not FDA-regulated medical devices, and have not been manufactured to the same degree of sterility as face masks or other items that directly interface with mucous membranes.  We make no claims regarding the safety and efficacy of the face masks in preventing any disease or illness, including, but not limited to COVID-19.
